Option Information
#1 Online Request:                                                                                                                                    
    • Go to www.irs.gov/transcript
    • Choose to "Get Transcript ONLINE (immediately)"
    • Follow steps on the IRS website to complete the request
    • Select Wage & Income Transcript for the appropriate year
#2 Paper Request:                                                                  
    • Form 4506-T, to access form click on 4506-T. 
    • Be sure to select OPTION 8 on the 4506-T form.
    • You can MAIL or FAX form. Instructions on 2nd page of 4506-T form
#3 In Person Request:                                                                   
    • Request an appointment by calling 1-844-545-5640 to schedule appointment in Modesto
