Homeless Shelter |
DETERMINATION DOCUMENT: Submit a determination document from the Director of an emergency shelter or transitional housing program funded by the U.S. Department of Housing and Urban Development verifying your you were an unaccompanied youth who was homeless. |

Transitional Living |
DETERMINATION DOCUMENT: Submit a determination document from the Director of a runaway or homeless youth basic center or transitional living program who determined you were an unaccompanied youth who was homeless or was self-supporting and at risk of being homeless. |

Homeless Youth |
DETERMINATION DOCUMENT: Submit a determination document from your high school or school district's homeless liaison verifying your homeless youth status. |
